How does an AI voice agent work on a real phone call?
Every AI voice agent is a relay between five components. The caller only notices the total time, so each handoff must be short.
A call arrives at a carrier such as Twilio or Telnyx, or through a SIP trunk from your phone system, and the audio streams in 20 millisecond chunks to the agent over a WebSocket or a LiveKit WebRTC room. Then five things happen on every turn:
- Turn taking. Voice activity detection and a turn detection model decide when the caller has finished. Pauses mid sentence are common, and ending the turn too early makes the agent interrupt.
- Speech to text. A streaming recogniser such as Deepgram, AssemblyAI or Azure AI Speech transcribes while the caller is still talking.
- The language model. A fast model such as GPT 5 mini, Claude Haiku or Gemini Flash reads the transcript and your instructions, then replies or calls a tool: check the diary, look up an order, create a ticket.
- Text to speech. A voice from ElevenLabs, Cartesia or similar starts speaking the first words before the model has finished the rest.
- Barge in. If the caller talks over the agent, playback stops and the agent listens again.
Speech to speech models such as the OpenAI Realtime API and the Gemini Live API fold these steps into one model. They sound more natural and respond faster, but give you less control and can cost more per minute. We use them where tone matters most.
The latency budget is the heart of voice AI development. People typically reply within about 200 milliseconds of each other in conversation, across ten languages studied (Stivers et al., PNAS, 2009). We target under about one second voice to voice: roughly 200 to 300 milliseconds for end of turn and final transcript, 300 to 450 milliseconds to the model’s first words, 100 to 150 milliseconds to first audio, and the rest for the network. Slow lookups get a short spoken filler such as “let me check that for you” instead of silence.

Inbound vs outbound calls: the rules that shape the design
Answering calls people choose to make is lightly regulated. Placing calls with an AI voice is not. The law in each country decides what an outbound agent may do.
We treat inbound and outbound as two products. An inbound AI receptionist needs AI disclosure, a recording notice and careful data handling. Outbound agents also need consent, calling windows, caller ID and opt out handling, and the rules differ by country.
- United States. In February 2024 the Federal Communications Commission confirmed that AI generated voices are “artificial” voices under the Telephone Consumer Protection Act (FCC Declaratory Ruling 24 17). Calls using them need the called party’s prior express consent, and prior express written consent for telemarketing, along with identification and opt out requirements. Several states add their own telemarketing rules.
- United Kingdom. Under PECR, automated marketing calls require specific prior consent to that type of call, while live marketing calls must be screened against the Telephone Preference Service (ICO guidance). In 2025 the ICO fined firms using avatar software that played recorded speech as automated callers, so we assume an AI voice agent making marketing calls needs automated call consent. Ofcom’s persistent misuse policy also covers abandoned and silent calls and requires a valid caller ID.
- United Arab Emirates. Cabinet Resolution No. 56 of 2024 on telemarketing, in force since August 2024 and supervised by the TDRA, limits marketing calls to 9am to 6pm, requires consent and registered local numbers, bans calls to numbers on the Do Not Call Registry and requires call records to be kept (Pinsent Masons summary).
Voice cloning brings its own consent question. We only clone a real person’s voice with documented, informed permission and a written scope of use, and never imitate a public figure. Tennessee’s ELVIS Act and similar state laws protect voice likeness in the US, and Article 50 of the EU AI Act requires people to be told when they are interacting with an AI system. Can an AI voice agent transfer calls to a real person?
Yes, and it should. Transfers trigger on request, on low confidence and on sensitive topics. After hours, the agent takes a detailed message instead.
Is it legal to use an AI voice agent for outbound calls?
Often yes, with conditions. In the US the FCC ruled in 2024 that AI generated voices count as artificial voices under the TCPA, so marketing calls need prior express written consent. The UK treats automated marketing calls under PECR as needing specific consent, and the UAE restricts telemarketing hours, requires consent and bans calls to numbers on its Do Not Call Registry. Inbound answering is far less restricted.
Can the voice agent speak both Arabic and English?
Yes. We build bilingual agents that detect the caller’s language in the first few seconds and continue in it, including switching mid call when a caller mixes Arabic and English, which is common in the Gulf. Accuracy varies by dialect, so we test Gulf, Levantine and Egyptian Arabic samples from your callers before choosing providers.
Can you clone our own voice or a staff member’s voice?
We can, with the person’s documented, informed consent and a written agreement on how and where the voice is used and how the consent can be withdrawn. Providers such as ElevenLabs require verification that the speaker agreed. Several US states, including Tennessee under its ELVIS Act, protect voice likeness. Many clients choose a licensed stock voice instead, which avoids the question entirely.
Should we use Vapi or Retell instead of paying for a custom build?
Use a platform when your calls are simple, volume is moderate and the built in integrations cover your systems. Platforms such as Vapi and Retell are quick to start and we often build on them. Choose custom engineering when you need deep integration with practice management or legacy phone systems, strict data residency, Arabic dialect tuning, or lower cost per minute at high volume.
